上來恭請三寶必賜降臨願展祥光俯垂鑒念現前一眾今欲進受五戒為出家入道之基本切慮人根淺薄魔障忽生故先歸命佛天願求加被庶使現前一眾正當傳授之時一心不亂萬境條然能所相冥授受無昧仰冀威靈決彰感應(念)。
