上來恭請三寶必賜降臨願展祥光俯垂鑒念現前一眾今欲進受沙彌十戒為出家入道之基本切慮人根淺薄魔障忽生故先歸命佛天願求加被庶使現前一眾正當傳授之時一心不亂萬境條然能所相冥授受無味仰冀威靈決彰威應(念)。
南無十方常住佛(法僧)護法諸天菩薩(各三聲尺一下坐)
